For warranty parts requests, please follow the instructions below.
This form is to be completed only by U.S. and Canadian wholesalers. Any others will not be accepted.
Technicians, please return the product to your wholesaler.
Homeowners, please contact your technician or return the part to the retailer from which it was purchased.
FOR BLOWER ASSEMBLIES, BURNERS, BURNER CONTROL ASSEMBLIES, IGNITION CONTROLS, MOTOR ASSEMBLIES, SWITCHES, VALVES AND WIRELESS THERMOSTATS, fill out the form below.
Canadian wholesalers will receive an email from asummers@ontor.com with an RMA and return instructions. If you don’t receive it, please check your spam folder. You will need these instructions and RMA number to include with the product before you can return it.
U.S. Wholesalers – Complete the form below and include a copy with the return product shipment (use the “print report” button on the final “thank you” page).

- One warranty form per customer invoice.
- Fully completed form and product must be included in the return shipment.
- Response to your claim will be provided within seven business days from the date of submission.
FOR ALL OTHER PARTS, YOU MUST CONTACT OUR CUSTOMER SERVICE DEPARTMENT FOR CREDIT APPROVAL.
- Both the model and serial number of the unit are required before we can process any warranty request.
FOR SHIPPING DISCREPANCIES, CARRIER LOSS, OR PRICING DISCREPANCIES, CONTACT OUR CUSTOMER SERVICE DEPARTMENT:
- For shipping discrepancies or carrier loss, credit must be requested within three business days from delivery with a signed POD stating shortage or damage.
- For pricing discrepancies, credit must be requested within 30 business days from shipment.
